IP Pack - GEERS claim form

The Department asks that you:

  • Distribute GEERS claim forms (PDF 1.3MB)  to each employee of the insolvent employer.
  • Ensure employees are aware of GEERS and the provisions in the GEERS Operational Arrangements that may affect their eligibility for an advance.
  • Ensure claim forms are returned as soon as possible to enable efficient processing.
