Forms for participants

If you are an existing NEIS participant, you can download the following forms and documents for use and reference during your period of NEIS Assistance.

If you are considering NEIS, a NEIS provider will explain what your obligations will be while you are receiving NEIS Assistance, and how and when you will need to use these forms.

Your Guide to the New Enterprise Incentive Scheme (PDF 421KB)
This leaflet includes general information about NEIS and your eligibility requirements.
Your Guide to NEIS Allowance (PDF 518KB)
This leaflet includes important information about your obligations while participating in NEIS.
NEIS Allowance Payment Dates Calendar (PDF 193KB)
This calendar includes dates that your NEIS Allowance will be paid (if applicable) and the dates that your quarterly Income Statements are due to be submitted to your NEIS provider.
NEIS Income Statement (PDF 118KB)
You are required to submit a completed Income Statement to your NEIS provider each financial quarter, by the dates shown on the NEIS Allowance Payment Dates Calendar.
NEIS Change of Circumstances Notification (PDF 99KB)
You are required to notify your NEIS provider immediately of any changes in your circumstances. Failure to do so may result in a delay in payments made to you or an overpayment of the NEIS Allowance, which may lead to a debt recovery being initiated by the Department of Education, Employment and Workplace Relations (DEEWR).
NEIS Participant Agreement (PDF 470KB)
This is the contract that you enter into with DEEWR prior to commencing NEIS Assistance.
